Improving Quality of Care for Medicare Members: the Connect for Quality Program (C4Q)
The Connect for Quality (C4Q) program is designed to enhance the quality of care and health outcomes for Viva Medicare members through a focus on disease prevention and chronic care management. This incentive program specifically targets primary care physicians (PCPs) participating with Viva Medicare, providing them with resources to foster greater provider engagement and member satisfaction.
Key Features of the Program
The program includes the support of a dedicated nurse clinician, who serves as a valuable resource for PCPs. This collaboration allows PCPs to earn bonus incentives by working closely with their quality nurse. Together, they aim to:
- Complete annual wellness visits
- Conduct preventive screenings
- Manage chronic diseases such as diabetes and hypertension
These efforts are designed to optimize health outcomes for patients.
Collaborative Approach
C4Q nurses collaborate with various disciplines, including Viva's care management and pharmacy teams, to implement effective quality strategies. These initiatives are structured around the calendar year and align with Stars and HEDIS measures, which are best achieved through proactive provider engagement. Key components include:
- Actionable Reporting: Providing PCPs with meaningful data to identify at-risk members and address gaps in care.
- Wellness Visits: Ensuring that members complete their annual wellness visits, including the Comprehensive Review Form (CRF).
- Preventive Care: Facilitating provider orders and scheduling for preventive screenings and necessary testing related to Stars and HEDIS measures.
- Outcome Monitoring: Tracking and trending outcomes related to Stars and HEDIS measures.
- Medication Management: Overseeing medication adherence, conducting post-discharge medication reconciliation, and ensuring smooth transitions of care.

The ultimate goal of the Connect for Quality (C4Q) program is to establish a true partnership among the health plan, providers, and patients. By emphasizing preventive measures and providing dedicated nursing support within physician practices, the program strives to improve the health and quality of life for Viva Medicare members. Through this collaborative effort, we aim to achieve better clinical outcomes and foster lasting improvements in patient care.
